The best way to get a family started is to help the parent get all of the necessary information ready to apply: (1) a link to our website: https://everykidsports.org/every-kid-sports-pass/ and (2) financial need documentation.
The parent MUST be the person to apply.
Documents and information a parent will need at the time of application:
- A Medicaid, SNAP, or WIC document that clearly shows that the child is receiving benefits. It must have the child’s name, the name of the benefit organization, and a date within the past 12 months. The document can be a photo(s) of an enrollment/re-enrollment letter or a benefit portal screenshot(s). There is room on the application to upload two photos/screenshots. A PHYSICAL BENEFIT CARD IS NOT ACCEPTABLE BECAUSE THEY USUALLY ONLY HAVE AN INITIAL ENROLLMENT DATE.
- Information about the program: league name (not an acronym), contact/email at sports organization, website URL, season dates, and cost to play.
