You will need the following information before you begin. Have it ready before you open the application.
- Qualifying document for each child
- Youth sports organization (YSO) name. Do NOT abbreviate.
- Sport your child is playing
- Name of a contact at the organization (coach, owner, director, etc.)
- Organization contact's phone number
- Organization contact's email address
- Link to the organization's website or social media page
- Registration start and end dates
- Total registration cost for the season
- Your grant request amount ($150 or less)
