Ensure your child's qualifying document has ALL the following information:

• Child's Name

• Date of Benefits (benefit date must be within one year of your application)

• The program your child is enrolled in

Examples of INVALID documents:

• The following will result in an automatic decline: 
  • Free and Reduced Lunch, CEP schools, 100% Free lunch programs, and P_EBT
  • Physical Medicaid, SNAP, or WIC cards 
  • Disability documentation 
  • Handwritten notes 
  • Documents missing information needed to qualify the child: 
    • The child's name must appear on the document
    • A date within 12 months must show the child is actively enrolled
    • The program the child is enrolled with must be present: Medicaid, SNAP, or WIC
  • WIC documents for a child who is not listed on the WIC form
