Unfortunately, because you typically receive a card only when originally issued from Medicaid, SNAP, or WIC, we will need another form of proof of current enrollment.
Check for your:
- Enrollment document
- Re-enrollment document (renewal letters are usually sent out yearly, or can often be obtained from your social worker or local benefit office.)
- Log into your online account/app and take a screenshot or picture of your child’s enrollment
Ensure the document you submit provides the child’s name, the program (Medicaid, SNAP, or WIC), and dates within 12 months from the current date.
