Medicaid, SNAP, and WIC do not send out a new card every year but my child is enrolled. What do I do?

Unfortunately, because you typically receive a card only one time from Medicaid, SNAP, or WIC we will need another form of proof of enrollment. 

Check for your:

• Enrollment document
• Re-enrollment document
• Log into your online account/app and take a screenshot or picture of your child’s enrollment

Ensure the document you submit provides the child’s name, the program (Medicaid, SNAP, or WIC), and dates within 12 months from the current date.
