The document you submitted does not include the name of the child you applied for.
Why does the document need my child's name?
We require your child's name on the document to confirm that they are currently enrolled in Medicaid, SNAP, or WIC. A document that only shows a parent or guardian's name is not enough, even if it is a valid government-issued document.
What if my document only shows the number of people in my household?
Documents that list a total number of household members without naming each person will not be accepted. The document must clearly show your child's name.
Tips for resubmitting:
- Some benefit letters have the parent's name and date on the first page and the household members listed on the second page. If that's the case, upload both pages — there is space on the application for two documents or photos.
- Try logging into your benefit provider's website or app and taking a screenshot that shows your child's name, the program name, and active dates within the last 12 months.

If you can provide a document that includes your child's name, you are welcome to reapply. Keep in mind that funds are limited each season and reapplying does not guarantee that you will receive funding. If you reapply, your application will go to the back of the line.
